Orthodontic Emergencies

Please call our office as soon as possible if you break or loosen any of your appliances. Please do not come directly to the office – by calling us, you will allow us to create a time to see you. Even if you have a regular appointment scheduled, call us immediately to notify us if you need an appliance repaired.

The following orthodontic emergencies and their treatments are listed in the order of the least severe to the most severe. Only the most severe emergencies may require immediate attention by an orthodontist. The majority of these are easily treated with a follow up with our office.

Teen Girl Sore Mouth

Food Caught Between Teeth

This is not an emergency, but can be a little uncomfortable or embarrassing for the braces-wearing patient. It is easily fixed with a piece of dental floss. Try tying a small knot in the middle of the floss to help remove the food, or use an interproximal brush or toothpick to dislodge food caught between the teeth and braces.

Discomfort

It’s normal for a patient to have discomfort for a day or two after braces or retainers are adjusted. But it can make eating uncomfortable. If your teeth are sore, stick to soft foods and rinse your mouth with warm salt water.

Mouth Sores

Some patients are susceptible to mouth sores. While braces do not cause them, they may be exacerbated by an irritation from braces. One or several areas of ulceration of the cheeks, lips or tongue may appear. This is not an emergency, but may be uncomfortable. To relieve discomfort apply a small amount of topical anesthetic like Ora-Gel directly to the ulcerated surface using a cotton swab.

Irritation of the Lips or Cheeks

Sometimes new braces can be irritating to the mouth, especially when the patient is eating. A small amount of non-medical relief wax makes an excellent buffer between the metal and mouth. Simply break off a small piece and roll it into a ball the size of a small pea. Flatten the ball and place it completely over the area of the braces causing irritation.

Protruding Wire

Occasionally, the end of a wire will work itself out of place and irritate the cheeks. Use a Q-tip or pencil eraser to push the wire so that it is flat against the tooth. If the wire cannot be moved into a comfortable position, cover it with relief wax. Please call our office to make us aware of the issue.

Loose Brackets, Wires or Bands

If the braces have come loose in any way, please notify our office to determine next steps. Brackets are the parts of braces attached to the teeth with special adhesive. They are usually positioned in the center of each tooth. If the loose bracket has rotated on the wire and is sticking out and the patient cannot immediately be taken to the orthodontist, you can temporarily fix to alleviate discomfort and prevent further damage. But take care to prevent swallowing or other injury.

To put the bracket back in place, use sterile tweezers to slide the bracket along the wire until it is between two teeth. Rotate the bracket back to the proper position, then slide it back to the center of the tooth.

Piece of Appliance is Swallowed

This is rare but when it does happen, it can be fairly alarming to the patient. Remain calm. If the patient is coughing excessively or having difficulty breathing, the piece could have been aspirated. If you are able to see the piece, you may carefully attempt to remove it. Do not attempt if you could cause harm.

If appropriate under the circumstances, examine the patient’s braces for problems that may result from the missing piece, such as looseness or irritation and treat as specified above.

If you are unable to see the piece and believe may have been aspirated, notify the orthodontist immediately.

Answers To Your FAQs From Dr. Garcia

At what age should I schedule an appointment for an orthodontic screening?

The American Association of Orthodontists recommends you should schedule your child’s first appointment by age 7. Orthodontic treatment and a child’s growth can complement each other. By timing orthodontic treatment to predictable stages of dental development, the orthodontist may be able to take advantage of your child’s growth and development.

Some problems that can be treated quite well in a growing child may require corrective surgery or permanent teeth to be extracted if treated after growth has occurred.

FAQs About Invisalign and Clear Aligners

How long do I have to wear the aligners?
Because each patient is different, treatment time with Invisalign varies. You could be in treatment for as little as 6 months or as long as 24 months – it all depends on the specific type of orthodontic problem you have and how compliant you are wearing your aligners and keeping your scheduled appointments.
Do I have to wear the aligners even at night?
Yes. In order for Invisalign to work as it’s designed, you must wear your aligners for up to 22 hours each day. You can remove them when eating and when brushing & flossing. You can also take them out temporarily for those special occasions such as interviews or school photos as long as you put them right back in afterward.
Do aligners hurt?
Aligners, such as Invisalign, don’t “hurt” but for the first couple days you may feel some slight discomfort. When there is soreness, Motrin or Advil (ibuprofen) can help ease the discomfort.
Are there certain foods I must avoid eating that might break my aligners?

Invisalign aligners are designed to be removed when eating. One of the greatest benefits associated with Invisalign is the fact that there are no food restrictions whatsoever. You can continue enjoying all your favorite foods including hard-crusted breads, corn-on-the-cob, etc. as long as you take your aligners out before eating, but be sure to at least rinse, if not brush and rinse, before replacing your Invisalign trays.

Any foods with color or food coloring (coffee, tea, red sauce, curry, sodas) can be on your teeth and slowly stain your attachments and/or trays.

Will my speech be affected?
When you first start wearing Invisalign, you may notice that your speech is not as clear as it normally is. This is nothing to panic about, as you simply must get used to wearing the aligners. Just spend a bit of time speaking or singing out loud while wearing your aligners and before you know it, your speech will return to normal.
Can I chew gum?
No, chewing gum is not a good idea because it will stick to your aligners and make a big mess.
Is it okay to smoke with my aligners in?
Because cigarette smoke can discolor and stain your aligners, smoking is not a good idea while wearing Invisalign.
How do I keep my aligners clean?
It is very easy to clean your aligners. Just brush them gently with a soft-bristled toothbrush and then rinse them off to keep them fresh & clean. To keep them extra fresh, we offer Invisalign Cleaning Crystals or Retainer Brite to freshen up the trays 1-2 times per week!
How often do I need to visit your office during treatment?
At the beginning of treatment, we may see you every 3-4 weeks. As treatment progresses, we can usually monitor your progress every 8-12 weeks. Each tray could be worn from 18 days in the beginning to 7 days towards the end of treatment, depending on cooperation and progress. Every patient’s needs are different, so that can vary from smile to smile.
Will I have to wear a retainer once I'm done wearing my aligners?
Yes. The day you are finished wearing your aligners is when you begin the retention phase of your treatment. This is when you will wear a retainer as directed by us so your teeth do not shift back into their original positions.
